Our PATIENT PORTAL allows you to access parts of your child(ren)'s medical records, including immunization records, lab results, visit summaries, growth parameters, allergies and medications. You may also request medication refills and access and download copies of Handouts recommended by your Doctor. You can also review your statements via the portal.
To access the Patient Portal:
1. Call our Office at (818) 713-9377 to get your PIN.

When your child turns 18, access to the Portal is automatically transferred to your child per the law - unless they give you access. Also the Portal access is tied to e-mail addresses - so if each parent or caregiver would like their own access to the Portal, they can register with their own e-mail address.
